This is why Rugby League's One Community has joined forces with Youngcare, to help change this dire situation. <br><br>Youngcare's committed to providing hope, possibilities and dignified care for all young people with high care needs, their families and carers. &nbsp;<br><br>Youngcare works to improve how the system currently operates, uniting the community, business and government to build and run the Youngcare Apartments for young people with round the clock care needs.&nbsp; The Apartments are state of the art and purpose built, that offer a unique model of care. The first of these is located in Sinnamon Park Brisbane, with the second due to be completed on the Gold Coast by 2010 followed by Sydney and Melbourne.<br><br>Other projects Youngcare is committed to delivering is Holiday Accommodation for young people with disabilities and their families to take a well earned break in a rarely found holiday environment, entirely disability friendly. <br><br>A Home Care grant program has also been developed to top up funding for those young people wishing to stay at home with loved ones.
